Location: Huntington Park, CA


 


Schedule: Full Time / Hours between 8am & 4:30pm.


 


Pay: $38 - $42  - Depending on Experience


 


Job Type: Temporary Assignment


 


 


 


Job Summary:Provide a psychosocial perspective to the interdisciplinary evaluation, assessment, care planning, coordination of care, and disenrollment processes of the PACE program. Interventions may include both individual and group modalities, family contacts, collateral contacts, participant and family education, assessment, counseling, mobilization of resources, identification and support of behavioral health needs, case management and advocacy, and discharge planning. Use knowledge of lifespan development, family systems, behavioral health, and social determinants of health to skillfully apply interventions that meet the unique needs of PACE participants and their families. The MSW collaborates as a core Interdisciplinary Team member to optimize the heath status and quality of life of the PACE participants.


 


Required Skills and Abilities:




1. Strong oral and written communication skills 


2. Ability to organize and prioritize goals and responsibilities.


3. Proficient with computer software such as Microsoft Office applications (MS Word, Excel, Access).


4. Self-motivated, flexible, and adaptable.


5. Excellent customer service and facilitator skills. 


6. Thorough, detail-oriented, able to follow through and work semi-independently. 7. Knowledge of DHCS and CMS.


8. Basic knowledge of HIPAA guidelines and requirements.


Required Qualifications :
Specifications / Experience:


1. Master's Degree in Social Work (MSW) from an accredited university is required 

2.Two or more years' experience in case management, social advocacy, and/or mental health, with the geriatric population. Field work/internship may substitute 1 year of experience. 

3. Bilingual Spanish/English, Chinese/English, or another second language is strongly preferred. 

4. A minimum requirement of a valid BLS certification or higher, following the American Heart Association (AHA) or the American Red Cross guidelines.
