Social Worker - MSW
Location: Los Angeles, CA
Schedule: Full time Monday-Friday 8-5pm
Pay: $35.58-40/hour - Depending on experience
Requirements: A minimum of two years of experience working with frail or elderly populations

Job Summary:
The MSW plans, organizes and implements social work services to participants and their families/caregivers. The Social Worker maintains a caseload of participants and works in close coordination with the Interdisciplinary Team (IDT). This individual serves as a liaison between the PACE program and the participants/families/caregivers. The Social Worker plays a key role in helping the frail elderly population to continue living independently at home.
Key Responsibilities:
- Participate in daily IDT meetings, conduct psychosocial assessments and care planning for each participant initially, semi-annual, annual or emergency
- Presents assessments at care plan conference with the goal of providing a foundation of plan of care develop by the IDT
- Collaborate with other IDT members to guide smooth care transition between settings (hospital, home, long-term, short-term care facilities)
- Provide community resources to the participants/families/caregivers as needed
- Facilitate participant moving between home and other placements
- Complete documentation in EMR in a timely manner and ensure accuracy
Required Qualifications :
Required Skills and Abilities:
- Must be able to lift and carry or otherwise move up to 25 pounds
- A minimum of two years of experience working with frail or elderly populations
- 1-2 years of experience of conducting psychosocial assessments, care planning and case management skills
Specifications:
- Education and Special Training - Master’s degree from an accredited university is required
- Certification - CPR/First Aid certified or able to obtain one within 30 days of hired
- Must have a current CA Driver License and clean driving record. Must maintain current automobile insurance
Experience:
- A minimum of two years of experience working with frail or elderly populations
- Bilingual: English/Spanish, English/Cantonese, English/Mandarin, English/Vietnamese preferred
- Experience working with multidisciplinary or medical professionals preferred
- Experience in EMR/EHR documentations preferred
