Location: Los Angeles
Schedule: Full Time / Hours between 8am & 5pm.
Pay: $42 - $55 - Depending on Experience
Job Type: Temporary Assignment
Job Summary:
Provide a psychosocial perspective to the interdisciplinary evaluation, assessment, plan of care and ongoing services.
Key Responsibilities:
Use knowledge of social systems and individual behavior to skillfully apply
Work Environment:
- Will be exposed to outside environmental conditions while traveling.
- May be subject to inclement weather on home visits
Required Skills and Abilities:
- Proficient with computer software such as Microsoft Office applications (MS Word, Excel, Access).
- Knowledge of DHCS and CMS.
- Basic knowledge of HIPAA guidelines and requirements.
- Ability to sit, stand, stoop, reach, lift (up to 10 pounds) for carrying out routine duties.
- . May be required to travel to attend meetings at other sites, to conduct home visits, and/or other job related functions

Bilingual: Spanish/English, Chinese/English or another second language is strongly preferred.
BLS/First Aid certificate is required
Experience:
Two or more years’ experience in case management, social advocacy, and/or mental health – with the geriatric population. Field work/internship may substitute 1 year of experience.
