Job Summary


The Speech-Language Pathologist (SLP) is responsible for delivering comprehensive speech therapy services to participants of Innovative Integrated Health (IIH). This includes evaluating and treating participants, developing individualized treatment plans, and collaborating closely with families, physicians, and interdisciplinary health teams. The SLP will maintain thorough documentation in participants’ medical records and coordinate with the rehabilitation team to design and implement effective therapy programs. Responsibilities also include discharge planning, assessing participants’ physical and communicative abilities, and setting rehabilitation goals. Travel may be required to support various IIH sites.


 


Essential Job Functions


Duties include, but are not limited to:


· Conduct thorough screening and evaluation of participants with communication, cognitive, or swallowing disorders, and 


  develop tailored care plans in accordance with regulatory and clinical standards. 


· Deliver rehabilitative, skilled, and medically necessary interventions for participants with communication, cognitive, and/or 


  swallowing impairments, ensuring compliance with clinical and regulatory guidelines. 


· Assess and adjust treatment plans based on participant progress, modifying services as needed, and making appropriate 


  referrals based on treatment outcomes. 


· Provide consultation, education, and counseling to participants, families, caregivers, and other providers regarding speech, 


  cognitive, and swallowing disorders. 


· Prepare and complete detailed discharge summaries, documenting all services rendered in line with regulatory and clinical 


  standards. 


· Accurately document all required information in the electronic health record system, ensuring compliance with company 


  policies and regulations. 


· Adhere to safety protocols when delivering care, ensuring participant well-being throughout treatment. Maintain confidentiality 


  of participant information.


· Performs skilled treatments at participant’s homes as necessary.


· Travel may be necessary to support all Innovative Integrated Health (IIH) sites.


· Adjust schedules as needed to meet the needs of Innovative Integrated Health participants.


· Attend and participate in staff meetings, in-service training, projects, and committees as assigned.


· Adhere to and support the center’s practices, procedures, and policies including assigned break times and attendance.


· Accept assigned duties in a cooperative manner; and perform all other related duties as assigned.


· Be flexible in schedule of hours worked, and in scheduling participants’ treatments to accommodate the participants’ availability.


· May require use of personal vehicle to provide therapy services in participant’s residence. If using a personal vehicle, a valid 


  California Driver’s License is required.

Working Conditions and Physical Demands

· Requires constant hand grasp and finger dexterity; frequent sitting, standing, walking and repetitive leg and arm movements, 


  occasional bending, reaching forward and overhead; squatting and kneeling.


· Ability to communicate verbally with an excellent comprehension of the English language. 


· Work is generally performed in an indoor, well-lit, well-ventilated, heated, and air-conditioned environment.


 


Experience


· Minimum of _2__ years of demonstrated successful experience in the quality healthcare field of work 


· Minimum of one (1) year of documented experience working with a frail or elderly population. 


 


Education and Certification 


· Current Speech Pathologist license in the State of California. CPR with First-Aid certification 


· CPR with First-Aid certification


· Is medically cleared for communicable diseases and has all immunizations up to date before engaging in direct participant 


  contact.
