Requirements to Apply:



  • Registered Substance Use Disorder (SUD) Counselor: Certification must be obtained from one of the agencies accredited by the NCCA and recognized by DHCS, such as CADTP and CCAPP. Candidates are required to adhere to all California Code of Regulations, Title 9, Chapter 8, and Title 9 requirements.

  • Minimum of two years of experience as a Case Manager working with populations facing low income, homelessness, prior incarceration, and co-occurring disorders.

  • Preferred: Bachelor's degree in Social Work, Criminal Justice, or Psychology.

  • Knowledge and experience with Drug Medi-Cal (DMC) and the Los Angeles County Substance Abuse Prevention Control (SAPC) system of care.

  • Proficiency in the delivery of trauma-informed treatment and evidence-based treatments for substance abuse, such as Matrix, Seeking Safety, CBT, Motivational Interviewing, and Nurturing Parenting Program.


Schedule: Full-Time; Sunday to Thursday 10 AM to 7 PM


Pay Rate: $19.00 - $24.00 per-hour


 


Responsibilities:



  • Execute routine care coordination following established SAPC policies and procedures, operating within the defined scope of education, training, and responsibilities.

  • Deliver gender-responsive and trauma-informed care coordination services.

  • Conduct initial assessments upon client admission to identify immediate care coordination needs and develop a care plan with specific timelines for each requirement.

  • Address care coordination challenges that have contributed to drug and alcohol use.

  • Identify immediate barriers preventing clients from staying in treatment and provide intensive care coordination for those areas.

  • Provide care coordination services for children and clients enrolled in the program.

  • Meet with new admissions twice a week initially, transitioning to once a week when clinically appropriate.
